Care Coordination

Care coordination between two or more providers is essential for good member care and continuous treatment. Every year, PerformCare surveys providers to monitor and improve the continuity and coordination of the care our members receive.

Providers are asked to provide PerformCare feedback on:

  • How they or other providers are jointly working to coordinate member treatment
  • Whether key member health information is shared with other providers
  • How discharge plans are shared among providers
  • If providers are reaching out to PerformCare when they are aware of barriers in treatment

Good communication with providers is the foundation of meeting our members' needs, especially during transitions. Effective collaboration positively impacts our members' recovery and is the key to keeping them stable and in their communities.

The 2025 Care Coordination Survey results

Here are the 2025 takeaways:

  • The total number of participating providers was 40, which was lower than the total number from 2024 (n=57). While many levels of care had a lower number (MH & SU PHP & IOP reduced from 13 to 5), SU IP had only one participating provider.
  • More intensive levels of care (MHIP, RTF, and FBMHS) are getting both sufficient and timely information the majority of the time, while the less intensive outpatient services report this as occurring most of the time (50-79% of the time).
  • PerformCare will be updating some questions next year, in particular the question, “Do the majority of other providers, such as outpatient providers, Targeted Case Managers, in-home service, respond to your outreaches within two business days?” as responses indicated that providers were confused on how to answer this question. 
  • Information from the responses, including the comments, will be shared internally at PerformCare to guide interventions to address these comments and hopefully assist in providers getting better care coordination, both with PerformCare, and each other; with the ultimate goal of improved outcomes and satisfaction for Members
