Transportation Services Assistance

PerformCare understands that it is sometimes hard to get transportation to and from your appointments. If you don't have a way to get to your mental health or substance use problem appointment, call the member contact information for your county. They will connect you with the Medical Assistance Transportation Program in your county.

Medical Assistance may also help you get transportation to appointments. The PerformCare Member Services representatives are happy to help you find out. You can also call them yourself. If you have a special need, be sure to let us know when you call.
