Online Personal Health Records

PerformCare wants you to take an active role in maintaining your health. One way to get a head start is to create a personal health record (PHR).

A PHR gives you a place online to track your health information. It may include your psychiatrist, family doctor's contact information, and the dates of your office visits. You decide with whom to share your PHR.

Why should you consider using a PHR?

A PHR can help you:

  • Help doctors help you: Many PHRs allow you to share information with your doctor. Your PHR can let them know if they need to pay special attention to a specific health issue noted in your record.
  • Make better health decisions: Have you ever had an allergic reaction? How about a medicine that didn't do the trick? Keep track here. Then update your doctor at your next visit.
  • Save energy: No more calling all of your doctors to find parts of your health care history. With a PHR, all of your information will be in one easy-to-find place.
  • Save time and money: Know the dates of your last doctor visit. Avoid repeat trips or tests.

What do you keep in your PHR?

It's simple. Anything that you believe is important to your health should go in your PHR, including:

  • Allergies.
  • Chronic diseases or mental health diagnoses.
  • Family health history.
  • Illness and hospitalization dates.
  • Imaging reports, like X-rays or CT scans.
  • Lab results.
  • Living will or advance directives.
  • Medicines and dosages.
  • Shots and vaccination dates.
  • Surgery dates.
  • Your doctors' names and phone numbers.

Choose a PHR

Are you ready to get started? Apps can provide easy access to your PHR when you need it.

  • Check your smart phone for a free app that came with the phone.
  • Check your cell phone under the app store for free apps that have a high rating.
  • Check with your doctor’s office or local hospital for their free apps.
  • Ask family and friends if they recommend a free app.

Remember, you don't have to use any of these PHRs. Keep them in mind in case you want help with tracking your health records.
