Care Management Referrals

PerformCare welcomes providers to refer members for additional support from PerformCare clinical care managers. Care management programs link members with PerformCare clinical care managers who assist members with coordinating care and linking to services that best meet their needs.

If you have a member who is struggling to connect with PerformCare services or who you feel would benefit from additional support from PerformCare clinical care managers, please use the secure contact form to refer the member to the care management programs.

Please use the comment section to note if the member needs an interpreter and what language the member speaks. Also note if the member is deaf or hard of hearing, so the care manager can use the PA Relay when they call the member.
