PerformCare is the behavioral health plan for our HealthChoices members living in Cumberland, Dauphin, Franklin, Fulton, Lancaster, Lebanon, and Perry counties.

About HealthChoices

• It is a program put into place by the Commonwealth of Pennsylvania's Department of Human Services.
• Requires those enrolled in the Medical Assistance program in certain counties to enroll in managed care plans to help them get services.
