2019 Care Coordination Survey Results

Coordination of behavioral health care goal

The total number of number of participants was 59. This was in part due to the removal of the mental health outpatient level of care in the survey process. However, there was a significant decrease from last year’s participation rate. Several attempts to encourage participants in all levels were made. Because of the low number of participants for most levels, rates should be interpreted cautiously.
 
2019 takeaways:
 
  • Except for substance use inpatient (long- and short-term rehabs), in general there remains an issue with obtaining sufficient, accurate, and timely clinical information when the member is first admitted. Previous efforts to improve clinical information were taken with some referral sources last year. This will remain a focus this year. 
  • There is a slight decrease in contacting other providers for ISPT meetings or treatment team meetings in all levels of care.  
  • There was a slight increase in contacting PerformCare about barriers or clinical care issues. 
  • In general, three levels of care — MH IP, RTF, and TCM — coordinated well with other providers concerning aftercare. SU IP improved slightly from last year’s rate. There is still need for improvement in the remaining levels of care. 
  • Areas of opportunities to improve care coordination and collaboration between providers will be taken to improve next year’s rates and efforts will be taken to improve the number of participants to ensure valid and reliable results.
